HomeBASE Coordinated Access & Assessment plays a functional role in demonstrating what is being done to end homelessness in Lethbridge; providing a seamless entry into the Lethbridge Housing First programs, while following specific inclusion and exclusion criteria for each of the programs.
The intent of the HomeBASE Team is not to replace the responsibility of the existing agency mandates, rather working with each Lethbridge Housing First Program to ensure warm transfer and program quality improvement.

Overview of Services
Participants who seek to access housing services through HomeBASE should come prepared to work with the Intake Workers through an intake process. This process typically takes 30-45 Minutes to complete and allows HomeBASE to provide the best possible referrals for all participants. In cases where referrals are made to the Lethbridge Housing First Programs, or other services, follow-up meetings will be scheduled to ensure that each unique situation is addressed to best of available resources and abilities. The process of referring participants in the best possible direction can take some time as follow-up workers and referrals are put in place to help participants to secure stable long-term housing solutions rather than emergency shelters.
HomeBASE does not have access to immediate housing for emergency situations, participants who find themselves in situation where immediate shelter is needed can seek assistance through the following resources:

Participants looking for assistance through the programs and services available through HomeBASE are advised to call and schedule a time to meet with an Intake Worker. When scheduling an appointment, the person making the booking will advise the participant on any steps to be taken prior to the meeting as well as any information that will need to be brought into the appointment.
